Turning Out For a Sport? Here's What You Need To Do!

 

Please download the following four files for Student Athlete Eligibility. Make sure you have all the required five parent and three student signatures on your forms before turning them into your school's Athletic Office.
 
  • Demographic Information (proof of residency in South Kitsap School District)
  • Proof of Insurance (company, policy # and parent signature)
  • Parent permission (parent signature)
  • Activity pledge (student signature)
  • Risk of Injury (student signature)
  • Risk of Injury (parent signature)
  • Medical history completed (parent/student)
  • Current Sports Physical (within the past 12 months)

ALL ATHLETES:  Washington State Law (HB 1824) regarding concussions.  In order to be eligible to participate in sports, every student athlete and their parent is required to sign and date the informational paper on concussions to the Athletic Office.  Forms are available in the Athletic Office (Item #2 above).  For more information, please click here.
 
SKHS Grade Policy - Grades from the previous trimester will be checked by athletic office or must be provided by athlete if home schooled, West Sound Tech or Running Start.
No fines - ASB and Participation Fees (show receipt of ASB payment).

 

ASB Card Cost:

Junior High - $30.00 (Once per school year)

 

Sports Participation Fee:

Junior High - $35.00 Competitive (per sport, per year)
 
Junior High Schools:
Marcus Whitman Jr. High does NOT require ASB/Sports fees to be paid until the first game/meet.
 
 
INSURANCE INFORMATION: The district policy requires that each student athlete have insurance coverage. One cost effective option is offered by UnitedHealthcare @ approximately $29/year. Online Enrollment available @ www.k12StudentInsurance.com Enrollment forms are also available in the main office at each school.
